Patient Information
Your First Appointment | Patient Forms Upon arrival, please report to the reception desk. On your initial office visit, you will be asked to fill out a patient information form before you see the doctor. You are responsible for bringing any referrals and making any co-payments required by your insurance carrier at the time of your visit.
Please download and print the patient forms listed below before coming to your appointment.
[PDF help] At the conclusion of your visit, you will be given the total charges for that day including the cost of laboratory tests and x-rays done as part of your examination. We will also inform you of any outstanding charges for hospital care if you were treated in the hospital by a member of our group.In an effort to reduce the cost of care, we request payment at the time of service. At the present time, our practice participates with the following health insurance plans:
Please remember that insurance is merely a way to reimburse the patient for doctor's fees and is not a substitute for payment. If you have any questions regarding our services or fees, please feel free to discuss them with us. If unusual circumstances necessitate special arrangements for payment of our bill, please bring this to our attention. We will be happy to work with you.
Appointment Scheduling, Changes, and Cancellations Appointments are scheduled between 9 a.m. and 5 p.m. Monday through Friday and are scheduled by calling the office that you prefer. Should you find it necessary to cancel or change an appointment, please notify the appropriate office as early as possible. Occasionally, the doctors' schedules and hospital emergencies necessitate a change in your appointment. When this occurs, we will do our best to contact you to avoid an extended wait or unnecessary trip on your part. Please be aware that patients who do not give us at least 1 business day notice of cancellation will be charged a fee for missed appointments. This fee is $125 for new patient consultations or pulmonary function laboratory appointments, and $35 for office followup visit appointments. Such fees are not covered by health insurance, and will be your responsibility During regular office hours our staff will notify the appropriate doctor and he or she will return your call. When the offices are closed, dialing the office phone will automatically forward your call to our answering system, through which you may contact the physician covering for emergencies. In a severe emergency, call an ambulance or go to the nearest hospital emergency room. The staff there will contact the physician on call. All prescriptions and renewals should be requested during regular office hours. Please have the name and number of your pharmacy and the name and dose of medication handy when you call for a renewal.
